Pharmacist Check-Up Program
PCP is also known as a "brown bag". A pharmacist reviews all your prescription and over-the-counter medications as well as vitamins, minerals, and herbal supplements. Due to the increased frequency of individuals using multiple physicians and multiple pharmacies a complete review of medication may help reduce the risk of drug related problems.
A pharmacist check-up can be a valuable tool to assess the effectiveness of your medication regimen. In addition, there has been increased awareness and use of natural remedies such as vitamins, herbs, and other supplements. These supplements may interact with other medications or disease states or some may contain the same ingredients. In a pharmacist check-up an evaluation is made concerning any drug interactions, cost savings, therapeutic duplications, and determination of whether the desired outcomes of drug therapy are being realized.
At your request this information may be made available to your
physician(s) and/or other health care providers.
The Components of the Pharmacy Check-up Program include:
- Detailed medical/medication history
- Review of all current medications and supplements
- Answers to your questions regarding your medication
- Identification and resolution of your drug therapy problems
- Written report for your records (a copy of this report may be sent to your physician or other health care providers at your request)
- Discussion of cost-effective therapies
